Make sure that the graphic file (prefferably in .bmp or .pcx format) uses the 256 colour Doom palette (a 16 million colour image that only uses the Doom colours still won't work).

You'll also want to make sure that the image is 256x128, or something like 256x240 if you're using ZDoom or Legacy.
