A hacky vanilla way to do it is to use the "reduce health and exit when dead" floor type. However in certian levels (? definitely in doom 2) it exits when your health gets very low, not when you are dead. You could either...
1- Have a "voodoo doll" (another start position which produces a static 'player' that can be hurt, which takes off your health) surrounded by barrels off the map, in a room JUST higher than the barrels. As you walk into the "reduce health and exit" area the crusher activates and explodes the barrels, so you'll die instantly and also exit.
2- A far cheaper way is to just use the "reduce health and die" floor then make the player start 'stuck' to a barrel in the next level. They'll have to shoot the barrel to escape, get killed and re-start, this time with a pistol but enough health to survive the explosion, pick up some medikits nearby, and carry on
3- You could also make an object which explodes like a barrel when it sees the player. Have an "exit when dead" floor in a dark room and make the player teleport into the room, which is also full of the instant-explode barrels
