I think I remember that book. I don't think that id software actually made the levels though, I think it was the authors of the book...

There's been about a billion posts about the Lost Episodes of Doom. It wasn't made by ID, it was made by a level author named Christian Klie and a few of his friends. I bought the book/level combo in 96 or thereabouts. The levels aren't anything spectacular, though.
