Problems with 99ways.wad

can someone make a UV MAX demo of Map 03 of the "99 ways to die.wad" ?
The only teleporter of the map does not work on my laptop for some reasons.

It looks like the teleport destination is missing on skill 4 (and 5). It works on skill 3.

For many early maps, playtesting tended to be done more on HMP than on UV, so quite a number of maps had issues like this. Indeed, in the txt, the author says that he geared the wad mainly for skill 3, and it looks like he was rather haphazard with UV.

It doesn't work because the teleport destination doesn't exist on UV due to a mapping error. So the map is simply impossible to finish on UV unless you edit it to fix this problem.

Since you guys are at it, do you guys know where is the last secret on map 03 of 99ways?
