Frankly it doesn't get more extreme than noise. You just strip away all music, and are just left with pure distortion, energy and shouting.

What happens when you use a guitar synthesizer as an actual synth (Which isn't its purpose) in Musique Concrete style and you put some death metal howls over it.

 

Then you get Xynfonica.

Electronic *core genres can get pretty extreme, especially the speedy ones. Also, noise and it's variations.

Danger Music. Not really music in a traditional sense, but instead seems to be a performance art that puts the performer and listeners in danger. For example, driving a bulldozer through the venue.

Brutal Death Metal, Grindcore, and Black Metal seems to be the teenagers choice when they start to experiment with their ear taste. But that music gets repetitive as hell, and almost all bands tells the same. In my case, i change my music taste almost all the time as i like to experiment new things and types of music. My favorites bands are The Pillows and The Back Horn, the first one its not extreme at all, and the second its a lot more catchy than extreme but its a lot harder. But i like to hear some experimental stuff/avan-garde jazzy thing, shoegaze, funk-jazz, P-Funk, Nintendocore, folk-bluegrass-gothic-country, murder ballad, stoner-sludge, post-hardcore, krautrock, japanese progresive/psychedelic rock, chilled-out electronic, j-hip hop, and just weird... i can put a lot more but its ok by now. All of them are extreme in some kind of sense, but not in the most used form of ''heavy and violent type of metal''.
If i put play in the first song of my winamp list today, it will stop playing around mid april... so yes, a lot of music. Like 500gb of it.

I don't know if this could be interpreted as extreme, but this album is one that i had the joy to get in my hands, a few years back, for only 5 bucks.
This songs is pretty extreme as it takes the Sister Rays approach by The Velvet Underground, and take it to eleven!

I don't really accept extreme as a label for music because it clearly becomes a very shallow designation in the face of actually just spending time listening. Sure I can put a Whitehouse record or something on and it's a little harsh at first but then after a few minutes it just becomes basically ordinary sounds, even soothingly hypnotic in a way. I've never listened to a record that was sonically or thematically so intense that even on repeated listens it's still too much for me, which would be my definition of extreme. Making "extremeness" into a dick swinging competition amongst artists is always a bad thing because if one tryhards too much at it then the effect ends up wrapping around to the other end and becoming obviously maudlin and silly, because it's mostly bad theatre.

"Extreme" metal is hardly extreme as far as genres go nowadays. Sure, at its inception, it was quite the "out there" genre, but it still kept to some standards of musical theory and known instruments already utilized, tried and tested in rock music.

 

Then again, it all comes down to subjectivity. 

To me, an extreme genre deliberately takes musical theory concepts and throws them out of the window, or plays around with them to the point of parody; the most popular examples I can think of in rock/jazz sphere would probably be Zappa/Mothers and Capt Beefheart, along with the Residents. There were also probably a lot more avant-garde artists that were ahead of them earlier than them, in classical music especially, these are just a few popular examples I can think of off the top of my head. (an example of babby's first avant-garde would probably be John Cage, Conlon Nancarrow, Delia Derbyshire maybe?)

 

And as some people in thread earlier argued, being extreme for the sake of being extreme just kinda dilutes the meaning, and extreme metal/grindcore usually tends to be babby's first introduction to the Heavier Stuff(tm), and by God do these genres get stale sometimes (and this is coming from someone who currently listens to those genres exclusively)

Any old idiot can toss dissonant noise into a recording. Believe me; I am one of those idiots. But it takes genius to take the base concepts of musical composition to their extremes.

I've been on an Opeth kick recently, but as far as extreme metal goes they're pretty chill on the whole, I think the most extreme stuff I really like is Meshuggah:

...But if we're talking about extreme stuff in general (not necessarily metal), stuff that'll reeeeeally mess with yer head, then you can't go past Mr. Bungle's debut:
