The only thing missing is sourceport version number, I wrote a post here with some more details on the prefered standard for uploading demos (though the version number and category are the only notable things for you): https://www.doomworld.com/forum/post/2050460 I didn't check, but since you are using glboom remember to set complevel if you haven't. I've always been a console player, so there's a giant learning curve for me when it comes to using a keyboard.
