What wad is that? Is it a terry wad or one of those weird wads (like Sky May Be) or is it a joke wad.

You see, the 99 Ways to Die PWAD was never TITLEPIC'd, the picture in question was left completely unaltered. Well except for mine, someone at some point who used to play this PWAD thought it was good idea to create their own TITLEPIC.

And yet the person comes along and says "uh '99 Ways to Die', well okay lets get something with death on it and put it on the cover" and just don't even bother to do any other kind of research whatsoever, it's actually amazing. But the backdrop is like "Doom meets Chernobyl" at the checkout line. So yeah it's not only bad but it's completely misleading I mean it doesn't represent anything about the PWAD whatsoever except the title. You could've stuck "Doom II" on there somewhere but no and this is what you get, and as such it's freakin amazing I kind of love it.

@Eggman07 mentioned "Vanilla as Fuck"; diving through the assets for "VaF Relived" (seems to be the latest?) I did find a very similar SSG sprite, but the pistol sprites are different. However, the sprites themselves aren't offset that high up, maybe that's done in-game?

Regarding the pistol sprites, I'm pretty sure those are the Press Beta pistol sprites, offset upwards near the center of the screen. The Press Beta sprites in general are "extended", although said extended portions are not visible with the used offsets anyways.
