It's from the Doomguy's haircut thread -that one used to be choke-full of "cursed images" anyway... more specifically, that "floating Doomguy's head" sprite pack is from this ZDoom mod.

It's from KDB.WAD, a 1994 predecessor to the 2xbigger series of Doom PWADS.

I've never watched Star Trek, so this WAD actually has nothing to do with that puzzle.

This is just a crap Doom WAD (and .DEH) made by me when I was 12 years old.

The metal texture is a custom texture I made for the WAD that was meant to resemble the metal scaffolding commonly seen on many different buildings, especially in my homeland New York City.

That gives me ideas for Death Star-esque endless vertical shafts, but of course with Doom 2's tetanus promoting base textures.
