It comes with DOOM or DOOM II... hmm, maybe not with the newer versions? Isn't there a link to it in the FAQ section here? It should also be in the idgames Archive. Finally, you can get it from the shareware ZIP, like doom19s.zip.

That FAQ is waaaaaaaaaay out of date. Pretty much all that's still of use as far as I can remember is the info about the monsters/weapons/maps. Maybe my memory fails me though.
