Are there any item placement differences between the Doom 64 difficulties, besides the blue keycard in map 1?

In map 1 the blue keycard is next to the switch, instead of in the room opened by the switch, on the default difficulty, which makes me think the default difficulty is not canon, which is a very strange decision on midways part. Are there any other major non-enemy count differences like this?

What I mean is that the game is not by default designed around that difficulty. Because that cubby was designed specifically to house the blue key, and it was altered for the bring it on difficulty
