monster and item first appearences

...in all commercial iwads. Not zombies or imps (a bit obvious) and stimpacks etc, but all other monsters, invulnerability, invisibility, soul sphere, etc. I remember seeing a faq for doom 2 only that had first monster appearances, ages ago. Also, the official 6.666 faq seems to have nothing like this in it.
