You do need to find the majority of secrets if you're playing this way on UV, however (at least I do). The shotgun near the beginning (just in front of the elevator, next to a splattered corpse) doesn't spawn on Hard difficulty, though you can steal one from the shotgun guy just around the corner. It'll leave you a bit short on ammo in the beginning as you don't get the shells from the missing weapon pickup, but you can scrounge up enough ammo to get by. Not sure exactly what the cause was, but I think someone had used the word fuck or fucking in the thread I was reading. Or it might have been something like anal, who knows?
