to damage sectors or not to?

i have 23 doom dm levels. i think i've done well to design each level according to the theme and flow of attaining items. but are damage sectors bad? it's not like you'll lose 50 health just for getting the plasma gun or armor. it's closer to about 3-25 health. it's not in all my levels either.

so i'm asking- why shouldn't there be a slight penalty for getting the bfg, pg, or armor? those items are somewhat overpowered. yes, there's strategy for getting those things. in many levels it's all about who gets those things first, because they can start to dominate the whole game as long as they keep getting those items over and over again.

should i be worried that all 23 of my levels will get me a bad review because 5 of them have damage sectors?
