Levels and stuff

I'm for the idea of a single continuous level with transports, real and/or interdimensional (surface to hell and vice versa) available along the way. I also would like there to be nice rest areas suitable for saving without being in a panic. :p

Imagine this scene: You are driving yourself to a clinic you passed by a few hours earlier because you need to have some wounds tended too, when you notice in your rear view mirror, an imp rise up behind you in the back seat. WHAT DO YOU DO? :o
