Deathknights of the Dark Citadel secret level

Ok, so I've pretty much scourged every possible pit, nook and cranny of all five levels with the wings of wrath and I still can't find that last damn gem! How do you get that thing?

For some reason I thought it's knights...meh. And I want the secret level for hub 1, not 2.
