The beginning of the file tells you the offset, in bytes, to the start of the directory, and how many lumps are listed in the directory, so you know the directory is (x*16) bytes long. The directory seems to generally be placed at the end of the WAD file, but this is just convention. Within the directory, each 16-byte entry gives you the offset to a lump / how large the lump is in bytes / the name of that lump.
