It probably is, I never really played Quake that much, so I must have just assumed it was a Hell Knight from Doom 3.

What's weird is I actually do. The two I remember best were the disks for Starcraft (first one) and Caesar III.

The disk for Rise of Nations I remember somewhat and it (and its box) had a neat design.
