The thing is, we have no idea (AFAIK) how old Olivia was when she took her post at Mars - she could have been (from the in game portrait) be early to mid thirties.
Samuel mentioned decades of research, add in the climate of Mars and the artificially produced gravity, air etc. No wonder she looks, well, fucked. I'm guessing she has just aged badly due to the Martian atmosphere, and it's probably given her terrible ailments such as arthritis, muscle atrophy etc.
The demons saw her wanting to be strong and pretty again, and prayed upon this by spoon feeding her the bullshit to get what they want.

Just my theory, at least.

The lore is that she had some bad form of scoliosis and she opted to get grafted to an exoskeleton thing. I'm not sure why that forecloses, like, putting on a jumpsuit, but whatever.

Someone who suffers extreme pain when standing or moving, but refuses to take pain killers because they could cloud her mind regarding further research does not seem likely to care about the newest fashion and how that exoskeleton that torments you 24/7 looks on you.

Well, considering they thought that Hell would be a good energy source and they have such wonderful slogans as "The road to hell is paved by the UAC" or something of that kind, I'm just going to assume that virtually everyone working in that God Forsaken place is just 100% brain-dead. They make the people who came up with Jurassic Park (In universe, obviously, not the writer) look brilliant in comparison.

Seems more like the suit takes it away.

If you ask me, making the suit that color is just awkward. I'm not gonna say anything on the design though.

The codex entry states that while she feels extreme pain due to the suit, she refuses to take pain killers as they would cloud her mind.

I'm feeling the same about the colour. If they had maybe used a darker tone or two different colours it would be easier to distinguish her from the suit and the exoskeleton. As of right now she just looks like one white/beige human-shaped mess.

I somehow get a "Dune Movie (David Lynch) Stillsuit (H.R. Giger - RIP)" vibe from her appearance. Maybe that's why I like it so much. It looks organic while at the same time it feels "wrong/artificial".

Doom is set in the future and by that point, you would have solved most any physical ailment through medical nanobots and genetic augmentation. But this is Doom we're talking about, so having outdated, unnecessary technology is a must. Props to the Dune reference btw.

You know what'd be funny? If it turned out the UAC was just way behind the times in terms of technology and the actual Earth was super futuristic. Given their brilliant idea to use Hell as an energy source, wouldn't surprise me.
